Body

For the safety and health of the community and our ministry, CHRISTUS Health is screening all visitors to our hospitals to help lessen the risk of infectious disease transmission among our patients, associates and guests. We appreciate your understanding as we ask questions to those visiting our campuses about their travel and health as it relates to screening for the COVID-19 symptoms, as well as take a temperature.

We will be screening our staff at each shift in the same manner.

Each patient is limited to one visitor, visitors must be 16 years old, and visiting hours will be 7 a.m.-7 p.m. (Please know this may be subject to change.) In addition, we only have one point of entry to the hospital through the Emergency Department, which will remain opened 24 hours a day, seven days a week.

Additionally, Health Information Management is currently closed to the public. If you need medical records, please use one of the following options:

• Using your MyChart login, send a message to Medical Records with the information you need and where you would like the re cords sent.

• Call 903-439-4045 to speak to staff members who can send your records to care providers for you. If you need the records for yourself, please request an authorization form, complete and return, along with a copy of your identification, to our office via fax or email. The fax number is 903- 439-4704. Our mailing address is 115 Airport Road, Sulphur Springs, Texas 75482, Attention: Medical Records.

• We are available by phone Monday-Friday from 8:30 a.m. to 5 p.m.

VIRTUAL VISITS

As part of our ongoing commitment to the health and wellness of our community, for Primary Care and Specialty Care, we are working to ensure patients may still see their provider for necessary appointments and we are doing this by implementing telephonic visits and virtual visits using telemedicine.

With a number of providers to serve you, we are happy to help with all of your healthcare needs. Call our office today to talk to our staff about your options for keeping your scheduled appointments, or with any health concerns at 903-885- 3181 or 903-606-DOCS (3627).

ELECTIVE PROCEDURES

Elective and non-urgent surgeries and procedures are those that can be rescheduled to a future time and will not significantly impact the patient’s health. In consultation with patients, providers will determine the best course of action to reschedule their procedure in order to meet the patient’s specific health needs.
